FWIW, I've been able to get IBM 00W0037 cards (rebadged MCX353A-FCBT, according to Mellanox) off of ebay for a $200 offer. That's the single-port version of the same card, but with only a low-profile bracket.

I bought a few of the dual port cards, they are the Dell ones. Very easy to flash back to stock image though.

flint -allow_psid_change -d mt4099_pciconf0 -i c:\firmware-filename.bin b

Those single port ones look nice as well.

FYI, I've had to reflash the IBM 40G cards using "custom" Mellanox firmware, more or less as described elsewhere on STH. They didn't want to talk to my 40G switch over QSFT until I flashed them, now they seem to work fine, at least in limited testing.

Flashing 8 cards is kind of a PITA, though, especially when the only convenient machine to use for flashing them only has 1 free PCI-E slot.
