I installed so many pcie cards with computer shut down that I thought it didn't make a difference if the power supply was was energized or not. This time the pins on the connector aligned just right, the magic smoke escaped, and now I have a dead motherboard.

ProDesk 600 G3 with CX4121, not Windows, but yes, shutdown.
The case has contact springs on the card openings, which push the bracket back pretty hard.

I was putting a Broadcom NIC into an X9SRL with the plugs still in the power supplies, and as soon as it hit the slot, the machine turned on.

No damage, but scary. Now, power cable gets pulled every time.

In ancient times it was recommended to press the power button again after unplugging the psu. To ensure residual power was all drained. I recall fans spinning for a couple of seconds!

Risks increased once standby (ie. Sleep, WOL) power was invented. And now we have USB ports that stay powered for charging.

On a related note I keep seeing videos where people are touching components and edge connectors with wild abandon. Probably while shuffling their feet on nylon carpets. Expensive modern kit is more resilient but the cheap Ali devices often discussed here may lack as much protection.
