ioMemory SX300-6400 Potentially failed?

Hi Guys hope you can help, I have been running this card fine for about a year now in my workstation however after an unscheduled reboot last night it's throwing up errors and is no longer accessible.

On boot the consistency check starts then fails with "ERROR(-5)-[4-0-1543G0123=0] advising me an error occurred and to use ioSphere.

If I try to attach within ioSphere I get the error "Result 35 (failed to read from or write to the device)

Doing a manual attach doesnt give me much more info:

C:\Windows\system32>fio-attach /dev/fct0

Error: Device '/dev/fct0' had an error while attaching. Failed to read from or write to the device.
Additional details about the error may be found in system logs.

Windows event viewer shows : "
fiowrn ioMemory SX300-6400 (4,0): Unable to save metadata. Reattach will require rebuild.
"

ACTIVE WARNINGS:
The bandwidth of the PCI slot is not optimal for the ioMemory.

Ignore the PCI-E error I have tried this in multiple slots to troubleshoot.

Any ideas? is the card fubar?

Ok I am now getting BSOD's with the card installed, removing it and uninstalling the driver appears to stop the BSOD's

I have managed to do a secure erase, when I try and attach I get the following error :

Error: Unsupported data format version on '/dev/fct0'.
Please extract old data using your previous driver,
re-format using fio-format, then attach and restore your data.

Formatting fails as well with this error :

Error: Device '/dev/fct0' had an error while formatting: Invalid input.
