To the card's credit, it was still linking and passing traffic but this was a scary sight to behold:
From what I could see, it seems like the heatsink pin failed causing the spring on the opposing side to push the heatsink off of the IC leading to more heat. The $0.01 part on this card that failed.
For those wondering, replaced by a dual 40GbE card.
