I ordered a few and received Finisar FTLX1471D3BCL. I did not tested the card nor the transceivers yet.
I asked Chelsio support for 110-1159-40 A0 (printed on the label) and 100-1120-00 REV C printed on the PCB to find out the model... They told me that according to the S/N (PT12345678 scheme) it is an T420-CR.
