I've had this problem on one of my hosts and 6.5. Is the USB drive you are attempting to install on only 8GB by any chance? I really don't think Esxi 6.5 supports drives less than 16GB. After i swapped out my 8GB for a 16GB USB drive I had no problems with installation. Hope this actually helps you.

I don't know the Rev # of the chassis. These are the rails that originally came with the server (the set I had to reuse) MCP-290-00057-0N

Chassis Model number is SC846E1-R900B, PSUs are PWS-902-1Rs. Currently I have (6) 2GB DIMMs installed, 3 per processor. Unfortunately I'm not including Rails since I needed to reuse them for a smaller 2U chassis that I installed in its place.
