I'm not familiar with this physical format. It says it's PCIe, but I don't recognize the connector.
What does this plug into? What connector, device, etc?

It is OCP3 standard. Mostly used at server grade boards. Signaling is PCIE, but mechanical form factor is said OCP3. OPC stands for Open Compute Project. This STH article might enlighten you:
